I believe Jean Watson’s Theory of Caringhas the greatest impact in my area of pediatrics. Nursing and caring have become synonymous in our culture and I believe caring is the essence of nursing. Professional nurses have an obligation to provide the best environment for healing to take place. The theory’s carative factors outlines principles and ideas that should be used by the professional nurse to create the best environment for healing of the patient and of the nurse.

            Watson’s Caring Theory and the Care of a Pediatric Cancer Patient by Rachel M. Parker and Crystal Lane-Tillerson was a great article to read and internalize.  The article presents an overview of Watson’s Caring Theory and applies it to the care of a pediatric cancer patient. A specific emphasis is placed on describing the nurse’s role in caring and presenting the expected outcomes of caring. Caring is essential to nursing practice and it can improve patient outcomes and experiences.  Not only does Watson argue that the theory describes caring within nursing practice, she maintains that her theory is unique in that it also captures the spiritual essence of nursing (Watson, 2012). I believe this to be true in my practice as a pediatric nurse. Caring creates valuable physical, mental, and spiritual connections between the nurse and the patient. This in turn can create meaningful relationships and positive patient outcomes. Watson’s theory is very applicable to present day nursing whether caring for a pediatric cancer patient or any other patient. All patients are in need of caring and Watson’s framework instructs the nurse on how to care (Parker & Lane-Tillerson, 2014).

Nursing theory and leadership was a challenging unit discussion. Several attempts have been made over the years to explain the characteristics of an effective leader. Leaders can be managers but not all managers can be effective leaders, making leadership an important aspect of effective management. In my personal experience, great management was something I did not experience until recently, thus finding it difficult to write about. By discussing nursing theory, leadership, and management in week 4, I was able to learn what qualities great leaders/managers have and what ways management incorporates theory into the nursing profession.  I also learned that by incorporating nursing theory into leadership, I will have a stronger knowledge base and develop a firm discipline whereby to provide quality of care through skills learned, and knowledge gained.

            The discussion that I found most useful to understand nursing theory and its application to my area of practice in pediatrics was the discussion on nursing theory and practice in unit 1. I was able to research multiple theories and discuss the value of theory to the profession of nursing. I was also able to identify ways in which I could use theory to improve my practice in my present role and future role as a pediatric nurse.

How to Create a Research Poster: Poster Basics 
Posters are widely used in the academic community, and most conferences include poster presentations in their program.  Research posters summarize information or research concisely and attractively to help publicize it and generate discussion.  The poster is usually a mixture of a brief text mixed with tables, graphs, pictures, and other presentation formats.  PowerPoint is a popular and easy-to-use software platform to make a poster.  It is part Microsoft Office. 
What makes a good poster? • Important information should be readable from about 10 feet away • Title is short and draws interest • Word count of 800 words or less • Text is clear and to the point • Use of bullets, numbering, and headlines make it easy to read • Effective use of graphics, color and fonts • Consistent and clean layout • Includes acknowledgments, your name and institutional affiliation

I just need to 2 responses to the Discussions, and about 100 words for each responses.

1. I chose the population of children under the age of 6 and I chose the Safety by Room checklist. I reviewed my family room, kitchen, and bedrooms; I was able to cross off each area on the check list. The modifications I would need to make would be in my bathrooms, backyard, and in my garage. In my bathrooms I don’ t have any anti-scalding devices on the faucets, locks on the toilet or cabinets, or non-skid rugs. In my garage I would need to install a carbon monoxide detector and in my backyard I would need to install a lock on my hot tub. Currently I do not have any children of my own so this isn’t a large concern of mine as there aren’t young children that live with me. I do have younger brothers that are under 10 years old so there are a few changes I would want to modify as they do come visit occasionally.

2. What is the role of Cal-Osha?  The role of Cal-OSHA is to set health and safety standards, enforce those standards, and provide education and outreach, certifications and licensing for all workers in the state of California.   As part of enforcing the standards, they are responsible to respond to complaints of unsafe practices and they cite and fine organizations based on the set standards.  They maintain and provide some general safety guidelines that apply to all places of employment in California along with some guidelines for specific types of industry such as the the agriculture or film industries.  Additionally, they also regulate the safety of those riding in public conveyances; such as elevators, trams, or amusement park rides.

    I selected a citation issued to Butler Amusements Inc. that operated rides for the Big Fresno Fair in Fresno, California in OCT 2013.  They were cited on 3 violations relating to the same thing, which was basically unsafe operation of 2 rids and “failure to maintain equipment according to manufacturers” guidelines.  These violations were considered”Serious Willful” violations because the owners knowingly removed or left off some parts that maintained stability of the rides. Their overall fine was $101,250.00 and it seems they fixed the issues. I am uncertain if that was a significant fine because the citation did not mention if there were any injuries or fatalities as a result of the violation. 

Throughout the year, and with California weather being so nice, one can encounter a number of these fairs popping up in most cities in California and all having some form of ride or another.  Many people flock to these fairs, which seem to be reminiscent of decades-past travelling circus, and reviewing this citation brings to mind something that I previously did not truly consider.  Although most people are aware that these events/fairs need to register and have permits for for holding their events, I for one don’t think that one would readily consider if the rides are being inspected or held to any major safety standard.  We all know that we want them to be fun and safe, and if something does go wrong with a ride, we would likely sue or complain to management. But to think that it would be a requirement to answer to the State’s Occupational Safety and Health Administration would not be something I would consider, until now.   Considering how many people these fairs attract, the money that is spent there (especially if you have children), and the potential for injury or harm, perhaps it would be good to ensure the public is aware and able to confirm/verify that at least the minimum safety standards are being followed and met, before we place our lives, or lives of our family members/friends in the hands of the fair operators.

 Small bowel series, Constriction of multiple segments of the small intestine Diagnostic Analysis The child’s small bowel series is compatible with Crohn disease of the small intestine. Intestinal absorption is diminished, as indicated by the abnormal D-xylose and lactose tolerance tests. Absorption is so bad that she cannot absorb vitamin B12. As a result, she has vitamin B12 deficiency anemia. She was placed on an aggressive immunosuppressive regimen, and her condition improved significantly. Unfortunately, 2 years later she experienced unremitting obstructive symptoms and required surgery. One year after surgery, her gastrointestinal function was normal, and her anemia had resolved. Her growth status matched her age group. Her absorption tests were normal, as were her B12 levels. Her immunosuppressive drugs were discontinued, and she is doing well.

The 57-year-old patient noted urinary hesitancy and a decrease in the force of his urinary stream for several months. Both had progressively become worse. His physical examination was essentially negative except for an enlarged prostate, which was bulky and soft.

Prostate ultrasound Diffusely enlarged prostate; no localized tumor Diagnostic Analysis Because of the patient’s symptoms, bladder outlet obstruction was highly suspected. Physical examination indicated an enlarged prostate. IVP studies corroborated that finding. The reduced urine flow rate indicated an obstruction distal to the urinary bladder. Because the patient was found to have a normal total voided volume, one could not say that the reduced flow rate was the result of an inadequately distended bladder. Rather, the bladder was appropriately distended, yet the flow rate was decreased. This indicated outlet obstruction. The cystogram indicated that the bladder was capable of mounting an effective pressure and was not an atonic bladder compatible with neurologic disease. The tonus limb again indicated the bladder was able to contract. The peak bladder pressure of 50 cm H2O was normal, again indicating appropriate muscular function of the bladder. Based on these studies, the patient was diagnosed with a urinary outlet obstruction. The PAP and PSA indicated benign prostatic hypertrophy (BPH). The ultrasound supported that diagnosis. Cystoscopy documented that finding, and the patient was appropriately treated by transurethral resection of the prostate (TURP). This patient did well postoperatively and had no major problems.

The leadership of any given workplace sets the tone for the entire operation. One key factor of this was presented by Anyati, Sudiro, Hadiwidjaja and Noermijati in their article addressing ethical leadership and its effect on the workplace. Their study found that an ethical leaderships system results in an ethical climate and prevents what they describe as “deviant behavior’ in the workplace” (2018). Leaders that use unethical means to enforce their policies will likely be met with undesirable actions from their employees. Leaders that create an ethical work environment and preform under their own ethical scrutiny are, likewise, met with ethical and pleasing behavoir in the workplace.  

     An example of this in my workplace from our Chief Operations Officer at Grand Peaks Medical. She conducts herself in an ethical way in all instances. When she has a child that needs to be seen for an illness, she calls and makes an appointment, sits in the waiting room and insists that regular operations and flow continue despite the fact that she is our boss, she could easily have her child seen first. This in turn has created an atmosphere of ethics in our office. We all have followed suit with scheduling ourselves and our family members. It has also deterred staff and doctors from giving important members of the community preferential treatment over another patient because of their social status. Our patients are all treated ethically regardless of financial, insurance or social status. We also all feel that we can go to her with concerns, because we know that she will handle them ethically, maintaining confidentiality, and will get back to us will answers in a timely manner.  

     A type of Transformational Leadership, known as inspirational, has been shown to be the one of the most effective in producing outcomes and a happy and healthy work environment. The Inspirational leader sets organizational goals and although he or she has high expectations of his or her employees, the leader is supportive and positive (Zindeldin, 2017). I found this type of leadership a key factor in good leadership because I feel it makes the organization feel like they are working as a team to achieve the same goal.  

     An example of this was my manager when I worked on Labor and Delivery. Our Nurse Manager was this type of manager and ran a very effective team. Each nurse meeting was organized to develop a team goal to improve patient safety. Sometimes this meant improving medication error rates or decreasing the call to cut time for a cesarean section. Not only did she give us helpful ideas on how to improve, she would spend time on the floor, working with us, taking care of patients. This gave her to opportunity to see what was hindering the goal, and to see what could be improved in way of policy that would help us meet that goal. We felt supported and felt that our manager truly cared for each of us.  

     Both of these leaders made a decision, as descried in chapter 7 of the course text, to be influential leaders. They both genuinely care about the staff and maintain a level of credibility among staff that provides them with a high level of respect from their employees. Being a leader is a selfless endeavor, and the best leaders aim to improve the lives of others not to improve their own status (Mashall & Broome, 2017).

Based on the financial outlook of your case/organization, make a recommendation to purchase or not purchase the system. You will need to draw on the material you have learned in throughout the class and assignments; as well as background information from your case description. Your final Financial Analysis report should be a minimum of eight (8) pages in length, excluding title and reference pages.

· Include an APA formatted reference page to include at least five (5) academic sources, such as a book or an academic scholarly journal, to support your topic. Your Presentation should be created in MS PowerPoint to include a minimum of seven (7) slides. You will also add a narration to your presentation that should be 15-20 minutes in length. Be sure to discuss your project components in a concise, yet effective manner. You should highlight key points of each component of the Financial Analysis Report, leading up to your EHR recommendation outcome. Include graphical displays (such as graphs or other pictorials) to add visual interest and support of your information. Use a professional tone and keep in mind that you are presenting to the executive management team; consider their interests as stakeholders.
