What is the geopolitical community in which you live

What is the geopolitical community in which you live? Why is it geopolitical?

My geopolitical community is Turlock, California. There is a big agricultural industry here and people’s sensibilities tend to lean conservatively, though there are small liberal pockets here and there. Often Turlock and Modesto (adjacent towns) are lumped together in terms of spacial community because a lot of people live in one and often work in the other. I live in Turlock but work in Modesto so I am involved in both communities socially and professionally.

What is a phenomenological community to which you belong? Why is it a phenomenological community?

My phenomenological community is very similar to my geopolitical one. If Modesto doesn’t count as part of my spacial community because I don’t live there, then I certainly feel like I’m a part of that community as well. There is also a significant Middle Eastern (specifically Assyrian) community in the Turlock/Modesto area and I certainly feel a part of them. While I was born and raised in the US my parents maintained a strong active cultural presence in our lives growing up.

What would some challenges and benefits be for community health nurses in providing care for different communities? For similar communities? For the same community? What are some possible solutions to the challenges?

Challenges to working in different communities would largely relate to communication and culture. Does the nurse speak the same language as the community in which he/she is working? That can be a big challenge to overcome. Also, practicing culturally competent care is another layer to add onto one’s practice and can be difficult at times. Ensuring that the nurse is not judgmental or condescending if practicing in a lower socioeconomic community than the one she is from is also important. We must treat all of our patients with respect and dignity. “Clinicians may also have to step outside of their usual therapeutic role to serve the client as an advocate” (Hope, Mocarski, Bautista, & Holt, 2016). This is specifically referring to community health workers working with people who identify as transgender, but it can apply to any member of any community. Nurses must be flexible and use all the resources at their disposal. On the other hand, some communities may have limited resources, so the nurse must make do with what he has. A benefit to all this may be a feeling of fulfillment or satisfaction in overcoming a lot of these challenges. Some possible solutions would be networking with outside resources or partially taking on that advocate or caseworker role (as long as the scope of the position allows) in ensuring the patients receive the best care possible.

When working in similar or the same community to one’s own, some of the challenges may still be there in terms of resources, however, one may experience a sense of kinship with these communities that make the challenges seem less daunting. The language barrier is usually not an issue with these communities and the nurse will already understand the culture of the members living in his/her own community.

Cultural diversity

Maurer and Smith (2013) define a geopolitical community as a spatial designation, either a geographical or geopolitical area, that is formed by natural or human-made boundaries. I live in Phoenix, Arizona which does not have a distinct boundary such as a river or mountain.  However, it has the Salt River Project which helped develop canal systems to allow for millions of people to live in the desert with access to water.  It is my geopolitical community because it is a geographical location and is also the state capitol.  One example of a man-made boundary in my community are zones which dictate the specific school our children attend. 

A phenomenological community is another way of looking at the community in terms of the citizens’ feeling of belonging or awareness of membership rather than a spatial designation. (Maurer & Smith, 2013). My phenomenological community is a middle-class neighborhood where residents are like-minded but have diverse cultures, ethnicities, and religious backgrounds.  We share common interests such as an active home owner’s association and neighborhood block watch.  There are numerous religious institutions surrounding our community. 

Cultural diversity would be a challenge for community health nurses in providing care to different communities and populations.  This is where the nurse needs to be aware of their own biases and beliefs while ensuring they are providing culturally competent care.  It is important to stay up-to-date and educated by such means as taking CEU’s.  Some communities may have limited means and the nurse will have to adapt to working with what is available.  Benefits include bringing the nurses insight, experience, and ideas to the community that may not have been thought possible.  When working with a similar or same community to the nurse’s own background, the community must still begin with an assessment by using the nursing process.  Assumptions should not be made.  While working in the same community, benefits include the firsthand knowledge of what issues are present in the community and what has worked and failed in the past.  Understanding of culture and diversity is an asset.  Addressing challenges needs to be done while prioritizing needs. According to Maurer & Smith (2013), physical, psychological, social, cultural, economic, and political considerations affect the appropriateness of strategies to solve a problem in any given community.  Taking time to assess the culture and needs of the community while building trust is key to addressing the people and the issues identified.

My idea of a healthy community

My idea of a healthy community would be a community where the inhabitants have good overall physical health as well as good mental health.  While we cannot get rid of disease, we can lower the rates of chronic disease.  A healthy community would provide a network for socialization, lower pollution, clean water, excellent and affordable primary care, and good schools.   In these community’s health promotion would be adopted by every community member and they would strive to maintain a lifestyle free from illness, injury and disease. 

Healthy communities have a lot in common.  These communities embrace the belief that health is more than the absence of disease, it includes the tools needed to allow people to maintain a high quality of life and productivity.  These communities are continually evolving to meet the needs of their inhabitants and provide the healthiest lifestyle possible.  They provide a healthy environment including clean air, clean water, proper sanitation, education and health promotion. 

There are many different views on what the ideal healthy community is and how to obtain it.  If I was providing care to a community that did not match my idea of a healthy community I would gather information, statistics, and data on the community to analyze how and where I could make changes and improvements. (Maurer & Smith, 2013) Take for example you find that there is an increase in children’s hospital admissions for asthma exacerbation.  Say I found that the children suffering lived in homes where the families could not afford adequate air filters for their air conditioning units.  I would look for grants or companies that would donate the right kind of filters to help these families keep their children healthier.

A healthy community

A healthy community may be one that has a lower risk of chronic disease and utilizes education and prevention strategies to promote health and reduce risk.  Community health nursing is a combination of nursing and public health practices that focus on prevention, health promotion, risk reduction, and well-being of a community or population (GCU, 2015). 

The Centers for Disease Control and Prevention (CDC) was attempting to address community health issues by the implementation of the Health Communities Program (HCP).  Unfortunately, this program is no longer funded.  However, the goal for the HCP was to partner with local and state officials to prevent chronic diseases, educate, and promote strategies to help people make healthy choices in all aspects of life (CDC, 2017).  Therefore, one example of a healthy community would be one that provides support by way of physical activity (safe and clean places to exercise and play), availability of healthy food (more farmers markets, fresh produce and meat vs. corner stores with less healthy selections), and smoke-free/drug free environments. There are many other things a healthy community could do such as workshops, cooking classes, exercise groups, or various types of clubs to get people (old and young) out and active. 

One thing that the CDC states is necessary to successful prevention of chronic diseases is providing the community with “the tools needed to achieve health equity and prevent chronic diseases” (CDC, 2017).  As a healthcare provider, I would utilize workshops or information sessions that include fact-based health information regarding prevention of obesity, hypertension, hyperlipidemia, diabetes, stroke, and heart disease or any other topic of interest within the community.  I would integrate how diet and exercise can help change these diagnosis’ and provide information and motivation to pursue healthier lifestyles.  Getting people involved in their own community can help change the community, as a whole, for the better. Community/public health nursing is the care provided by educated nurses in a particular place and time directed toward promoting, restoring, and preserving the health of the total population or community (Maurer & Smith, 2013).
